For Pre- Kindergarten to Grade 10:
1. PSA Birth Certificate (scanned copy)
2. Recent ID picture of student in white background (clear photo)
For Grade 1 to 10 only:
3. Good Moral Certificate or Certificate of Eligibility to Transfer; from previous school (scanned copy)
4. Final Progress Report Card or SF9 with LRN; from previous school (scanned copy)
5. Learner's Reference Number of the Student
For Grade 1 only:
6. ECCD report card from previous school (scanned copy)
7. Certificate of Kindergarten completion (scanned copy)
For Grade 8 to 10 only:
8. ESC ID Number (if student is an ESC grantee)
Additional for Pre-kindergarten:
- Updated baby book / health record
- ECCD Card (from Health Center)
- Growth Chart (from Health Center)
