This is the online form to place an order for a sheep to be slaughtered for Qurbaan or Aqeeqah purposes on Eid-ul-Adgha (Tuesday 20 July 2021 and/or Wednesday 21 July 2021).

Please note the form is designed to provide details for the slaughter of a single sheep. If more than 1 sheep is ordered, the same number of forms needs to be completed as the number of sheep ordered.
