Return to In-Person Learning Intent Form
This form is intended for families with students in grades 2-5 to commit to a model of learning for their child when we consider transitioning students to in-person.  The choice made for your child(ren) is binding through the remainder of the year.  You must complete a survey for each child.
 
At this time, we are beginning to plan for our students in 2nd grade through 12th grade to return to an in-person model of instruction.   The next cohort to return to in-person learning will be students in 2nd grade and 3rd grades.  The following cohort will be planned for 4th and 5th grades.  Finally, we will plan for the return of students in grades 6th through 12th.  
 
Our plan is to continue to offer in-person learning for each cohort Monday through Thursday from 9:00 to 1:30pm.  This model will follow the same schedule as the students currently instructed virtually.  This schedule may be adjusted to lengthen the instructional time available during the day.  Students in either in-person or virtual instruction may receive a new teacher assignment.
 
Please use this form to indicate your intent for your child to receive instruction in-person or virtual.  The data from this survey will be utilized to determine when and how the 2nd through 5th grade students will be able to return.

The school board will discuss and vote on this plan at the November 30th school board meeting.  Following the school board's decision, your child's school will communicate with you about the next steps.
 
We ask that parent(s)/guardian(s) complete the survey by November 19th at 5:00 pm to provide adequate planning time for school staff.
