IEHA District Meeting Registration Form
By attending one of IEHA's district meetings, you will earn 7 CEU's. Please fill out and complete this form. Once you have finished, submit your responses. If you have questions, please contact IEHA directly at 630-506-6770 or by email at iehaufmw@gmail.com to speak with a representative. For room reservations, please contact the office or call the hotel directly. The hotel room reservations will be available on December 19, 2022. The district meeting registration cost is $259 per person.

When paying by credit card, a $4.00 Credit Card Processing fee will be added to your total. No refunds will be issued up to 90 days before the scheduled event date and no transfer. Please note change of date/time is at the discretion of the facilitator. IEHA reserves the right to cancel ANY district meeting or class 14 days prior to the scheduled date.

If you are paying by check, make them payable to IEHA and mail to:   (P.O. Box 400 Westerville, OH 43086).

All district meetings will start at 10am in each district except Hawaii.  A vendor lunch or networking dinner is included in the registration.
