This form is intended for parish leaders to sign up for spots for the summer 2024 Love Begins Here local mission experiences. Parish leaders should sign up the number of spots they would like to reserve for a specific week of the summer. After this initial registration is complete, parish leaders will be sent a link to an individual registration form they can send to their participants.
By registering on this form, parish leaders are committing to the $10 non-refundable deposit for each student participant spot they register. (If you sign up for 10 student spot, you will invoiced for $100). Final payment of the total $125 registration fee per student participant will be due in May. Chaperones are able to attend free of charge thanks to in part to a grant from the Catholic Foundation of the Diocese of Green Bay.
Important Registration Information
LBH Middle School will be open to students who will be in 6th grade through students in 9th grade for the 2024-2025 school year (current 5th-8th graders)
We will be asking for a $10 non-refundable deposit on every spot you reserve in order to help plan for the most accurate numbers for the summer.
The total registration fee for participants is $125 per student. Chaperones are free to attend!
Parishes are expected to provide VIRTUS Trained and properly background checked adult chaperones over the age of 21.
The ratio of students to adults should be 8:1 and should account for both genders. For example if you bring 5 boys and 3 girls you should still try to have 1 male chaperone and 1 female chaperone for your group.
Adult chaperones are also expected to be able to transport in a vehicle the same number of students they are registering for their group. (If you register 9 students you will need at least 2 chaperones with access to vehicles that could transport 5 students in one car and 4 students in another)
After you register the number of spots for your group, you will receive a separate link to send to participants for individual registration.

To be added to the waitlist, please fill out the form below and select the waitlist option. Waitlists will be honored on a first come first served basis as best we can. You will not be invoiced for a spot on the waitlist and the deposit will only become required if you are given confirmed spots on a mission trip.
