In order to cultivate a just and generous expression of the Christian faith and to protect those communities that have been oppressed, Forefront Church does not tolerate any form of discrimination, harassment, targeting or abuse of members of our community. The purpose of this form is to provide a mechanism to report incidents that have been experienced or witnessed that contravene the values of Forefront Church. 

You have the choice to submit an incident report anonymously or you may include your name and contact details. If you decide to share your information, please know that your identity will be protected. Any incident that gets reported will be thoroughly investigated by the Forefront Church Leadership Team and will protect the privacy of all parties involved in compliance with local, state, and national laws and guidelines.
