London Youth collects personal information of young people with who we have regular contact. Please read the following information and all forms thoroughly before completing.

Data Collection Policy:

Why we collect your information:
All personal information included on the forms is collected to administer your participation on the Keeping it Wild programme.

How we use your information:
The information and data provided on the forms will be used to:
• Administer the programme and monitor our activities. Your personal responses on this form will only be shared with authorised project partners, evaluation consultants and the funder for reporting purposes (e.g. National Lottery Heritage Fund and evaluation consultants (Renaisi, Centre for Youth impact and Shephard & Moyes Ltd).
• With you or your parents’ consent we may:
o Use photographs, video and audio recordings of young people to showcase and promote the programme, London Wildlife Trust and project partners London Youth, Headliners (UK) and John Muir Trust.
• Register you with Open College Network (OCN) if completing an accreditation in Media and Journalism as part of the project (note: this will require your data to be shared with OCN)
• Support you with any needs that you might have (completing this information is optional)
• We will not sell or share your information with any other parties.

You have the right to request access to your data and for us to correct or delete your data by contacting us at the address below. You may withdraw your consent to us processing this data or request that we do not contact you by any of the methods below at any time. However that may mean that you will be unable to participate in some or all of the programme.

How we will store your information
The information provided on this form will be securely stored within London Wildlife Trust, partner (outlined above) and evaluation consultant databases. Your data will only be accessed by authorised personnel and authorised parties who are responsible for the maintenance and security of our digital systems. If any parts of this form are unclear or you are unable to complete it yourself, then please let us know and a different form can be provided for your parent or guardian to complete with you.
