Waiver- Waiver/Release Form.

This section must be read and signed before any student is allowed to participate in a dance class. Please take the time and read carefully!
I hereby waive all claims for injury, damage, or loss to my person and properly during my participation in Studio One Dance Center and release the director, principals, agents and employees of Studio One Dance Center from any act or omission. By signing, the undersigned and participants and/or his/her parents/guardians expressly adopts and agrees to be bound by the below waiver and release agreement. The signature of the parent/guardian is required in the case of all participants who are under 18 years of age (participants 18 and over must sign for themselves). The proper signing of this agreement on this form is a condition of the participation in Studio One Dance Center.
