Middle School Art Club Registration Form
This form is to register your student for Middle School Art Club.  A club leader will contact you the day before each club to provide you with a zoom link to participate.  Please complete this form for each student in your family wishing to participate.  By filling out the survey you are granting permission for your students to participate in our Middle School Art club.  Our club is open to grades 6-8 only, please.

NOTE: This class will be completing activities that will require the use of some basic art supplies. These supplies will be put together into a kit and distributed to all students who have registered by the deadline - one week before each registered club.
