Please fill out all fields to register for Music Performance Assessment.   Please mail a copy of the receipt with payment to Doug Farris by March 12, 2021. (If you are registering multiple bands, you will need to complete this form twice.  Please include both receipts with your payment.)  The address will be provided on the receipt.  The links for submitting files will be included in your receipt and must be submitted by April 9th.  There will be two file requests, one for audio/visual files and one for all paper work such as scores, rosters, etc.
