Students, in order to be contacted by a Student Services Team member for a Social Emotional reason ONLY, please complete the following form.

Student Services Team members will be monitoring the form and come and find you in class.

This form is monitored during school hours. If submissions happen after school hours on week days or over the weekend, this form is not monitored until the next school day.        

Parents, if you have a concern about your child, please contact their School Counselor or other Student Services Team member directly. Contact information can be found on the WHS Student Services website.
