NorthRidge Church has compiled a COVID-19 EVENTS PROTOCOL. These guidelines reflect those laid out by the BC Health Ministers Office as well as Worksafe BC. As we begin meetings in person, we ask you read and agree to the following:

HEALTH POLICY & WAIVER

By registering, you acknowledge and agree to the following:

You are not sick (coughing, sneezing, shortness of breath, etc).

You have not travelled from outside of Canada

You have not been advised by public health to self-isolate.

You will inform NorthRidge Church if you have contact with someone who has COVID-19.

You will sign in and provide current contact information.

You will maintain a 2-meter social distancing.

You understand that we prefer you wear a mask while inside the building.

Pick up and Drop Off

• Children/youth will be dropped off outside the activity room. Parents and

guardians are NOT permitted inside the activity room.

• Upon entering children/youth will be directed to use the hand sanitizer.

• Each household will have a pre-selected spot to sit.

• Pick up is at the outside door.

• Avoid close greetings such as hugs and handshakes.

If you are filling out this form for a minor (under 18 years old) you agree to educate and inform those minors of the policies in place.  

RELEASE OF RESPONSIBILITY
By completing this registration you are declaring that you understand and agree to release NorthRidge Church, its Council, and Employees, from any claim or action related to potential exposure or contraction of COVID-19
