Because students are 100% virtual this semester, students must have access to a gaming console or gaming PC to play Rocket League from home. Does this student have access to a gaming device and internet to play? The school does not have any gaming devices or gaming PCs for students at this time. *
Can this student attend practices and games up to two times a week on Wednesdays and Thursdays from 4-6 PM MST? *
If the student has played Rocket League before, what is their highest rank in any competitive mode? If not,  choose not ranked. *
As the parent/guardian, do you give your child permission to use our Discord voice and messaging tool for practices, games, and overall communication for our club? The app/website is also monitored by JMMS Esports staff. The other channels and servers are not monitored. *
As a student, are you interested in casting games and shout-casting live JMMS Esports Rocket League games on YT or Twitch? *
As a student, what do you think your biggest strength is when it comes to playing competitive video games? *
As a student, what do you think your biggest weakness is when it comes to playing competitive video games? *
JMMS Esports prides itself by demonstrating athletic sportsmanship in and out of the virtual arena. Do you pledge to practice good sportsmanship? This means no inappropriate behavior during or after matches. You must act as a model for all other gamers in the school. If student behavior is an issue, they will be asked to leave the team for the rest of the season.   *
JMMS Esports encourages their players to be "Scholar Gamers". We want our students to maintain a 3.0 GPA to play in competitive matches and a 2.5 GPA to practice. Can your student achieve this while playing this season? *
