2022 Summer Camp Registration Form
You can easily choose one camp, two camps or TAKE THEM ALL.  We are excited to offer a variety of styles to fit your dancers and tumblers needs this summer.  Summer Camp runs from June 1st - June 30th.  Pricing for the camps is listed below or you may contact a member of our staff for more information.  

Spots fill up quickly and could sell-out.  Don't MISS THIS!

Once you complete the form - Camp Charges will be added to your account (or a new account created for you for new students).  You may still adjust your camp choices afterwards.  Price is PER camp chosen or a discounted price for attending the Full Camp.

(*) Starlet KIDZ Camp June 6th - June 9th (ages 2-3 & 4-5 separate classes by age group)
     10 am class OR 6 pm class - one hour class time for 4 days = $55.00

(1) Choose one 4 day Camp = $85 (Recommended for Ages 6+)

(2) Choose two 4 day Camps = $170 (Recommended for Ages 6+)

(3) Choose three 4 day Camps = $235 ($20 Savings) (Recommended for Ages 6+)

(4) Choose ALL four - 4 day Camps = $285 ($55 Savings)  (Recommended for Ages 6+)

If you choose the FULL Month of Camps; all 4 styles - Tumbling, Jazz, Contemporary and Hip Hop, the discounted price is $285.  An EARLY-BIRD discounted savings of $55.00.

Once your form has submitted, your account will be charged accordingly.  You must make arrangements to pay for camp by May 24th.

This Discounted Price is ONLY GOOD UNTIL MAY 15th.  After May 15th camp price will increase to $95.00 each with NO multi-camp discount. Full 4-week Camp Price will increase to $380.

By signing this form, you agree to allow your student to participate in the above mentioned activity.  Due to current pandemic situations, masks may be required to participate.  Starstruck Studios Dance & Tumbling will abide by State and Local Health Laws at all times.  Our first priority is to the health and safety of everyone that participates and works with Starstruck Studios. By signing, you understand the inherent risks and choose to allow your student to participate of your own free will.  I agree to indemnify and hold harmless Starstruck studios its successor and/or assigns from all liability, claims, demands, losses, or damages on minors account caused or alleged to have been caused in whole or in part by negligence or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minors behalf makes a claim against Starstruck Studios, its successors and or assigns, I WILL INDEMNIFY AND HOLD HARMLESS from any litigation expenses, attorney fees, loss, liability, damage, or cost Starstruck Studios may incur as the result of any such claim. I understand that there will be no refunds given or make-up lessons scheduled for any classes missed by student(s).  I further understand and do hereby grant permission to Starstruck Studios LLC to use the image(s) of my child(ren) listed above.  Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Starstruck Studios website.  I agree that these images may be used by Starstruck Studios for a variety of purposes and that these images may be used without further notifying me.  I do understand that the child's last name will not be used in conjunction with any video or digital images for marketing purposes.  No remuneration will be given to any participant for images used or as referenced above. By signing this Release/Authorization I attest that I have read and understand the above terms of this waiver, and agree to abide fully by its terms.
