Thank you for deciding to enroll in Our Whole Lives at Community Unitarian Universalist Church in Spring of 2024. We believe that by educating young people about bodies, sexuality, and relationships in this comprehensive way, we are working to bend the moral arc of the universe towards justice.

This year, registration involves 3 tasks:

1) Fill out this Registration Form

2) The student's legal custodial parties must sign a paper Permission Slip, available at the Kickoff (due March 3rd).

3) Payment. You can pay electronically, or by cash or check.

We collect a lot of information in this form. The form has several sections, and you can't see them all from this start page. It activates my anxiety when I have to fill out a form like that! So, here's an outline of what the form asks you to do: Section Bloop asks you to provide contact information for your students legal custodial parties, and up to 4 other adults who are caregivers/share responsibility for your student; Section Bleep asks you to tell us about your student so our facilitators can be best prepared to welcome your student in our class; finally, Section Blop is about the class fee, and where you can learn more information about our scholarship program.
