Thank you in advance for filling out your end of event report. These reports serve as the reporting system for broken, malfunctioning or missing equipment, test and measure our payroll, staffing successes and opportunities for improvement, and most importantly, help us improve the quality of the products & services we provide to our customers. Please take your time when filing this report out and be as descriptive as possible.

NOTE: EOS reports are to be completed no later than the Monday at 8 am after your event. EOS reports MUST be filled out in order to be paid for that specific event.
