Because planning, executing and documenting work has become a primary function of the community health worker and much of that work now occurs online, this course features an expert from KC CARE Health Center demonstrating KC CARE’s care coordination system from Intraprise Health, including the documentation associated with planning, intervening, and following up. This will give a “look and feel” for such systems when a community health worker encounters them in the workplace and will be of particular interest to those who are interested in online documentation and best practices.
