MOTORCYCLE RIDER & PASSENGER POKER RUN REGISTRATION & RELEASE FORM

EVENT INFORMATION

The undersigned (on our behalf and on the behalf of our heirs, personal representatives, successors and assigns), for and in consideration of the opportunity to participate in a “Poker Run”, “Ride”, or “Activities” sponsored by Nam Knights of America Motorcycle Club, Tri-Base Chapter (Nam Knights of America Motorcycle Club, Tri-Base Chapter Inc.) and their respective officers, members, and volunteers (hereinafter the “RELEASED PARTIES”) releases and holds harmless the “RELEASED PARTIES,” ALL OTHER SPONSORS, INCLUDING AMERICAN LEGION EDEN-STANLEY POST 294, BACKWARD FLAG BREWING COMPANY, MICKEY’S PORT OF CALL, AND THE PLUMSTED GRILL and other participants on this motorcycle ride from any and all claims and demands, rights, and causes of action of any whatsoever which I now have or later have resulting from, arising out of or in connection with my participation in this ride.

This release extends to all claims I have, against the “RELEASED PARTIES”. Resulting from or arising out of their performance of their Chapter duties whether such claims result from negligence (except willful neglect) on the part of any or all the “RELEASED PARTIES” with respect to the EVENT(S) or with respect to the conditions, qualifications, instructions, rules, or procedures under with the EVENT(S) are conducted or from any other causes.

I UNDERSTAND THIS MEANS I AGREE NOT TO SUE ANY OR ALL OF THE “RELEASED PARTIES/SPONSORS” FOR ANY INJURY/DISEASE/DEATH RESULTING TO MYSELF OR MY PROPERTY ARISING FROM, OR IN CONNECTION WITH THE PERFORMANCE OF THEIR CHAPTER DUTIES IN SPONSORING, PLANNING OR CONDUCTING THE EVENTS.  I DO HAVE MOTORCYCLE LICENSE AND INSURANCE.

I AM VOLUNTARILY PARTICIPATING IN THE EVENT AND EXPRESSLY AGREE TO ASSUME THE ENTIRE RISK OF ANY ACCIDENT OR PERSONAL INJURY, INCLUDING COVID-19, AND OTHER DISEASE AND DEATH.

By signing (in person or electronic) this release, I certify that I have read this release and fully understand it and that I am not relying on any statements or representations made by the "RELEASED PARTIES" and do claim that I do have motorcycle insurance and a valid motorcycle license from the state which I am a resident of.  The activities of this event may be videotaped. As the undersigned, I agree that my likeness may be used in promotional materials and will hold harmless the “Released Parties” and agree that no monetary or any other consideration will be given for their use.

THIS RELEASE FORM MUST BE SIGNED BY RIDER AND PASSENGER.
