Dance Island Registration Form
TUITION: Tuition is payable in advance and due by the 5th of every month. If paying after the 5th, please add a $10.00 late fee to your tuition payment. Monthly fees are calculated and prorated over the school year, taking into consideration holidays and months with 5 weeks, and will remain the same for every month unless notified by Dance Island.

* An annual $10.00 registration fee is payable every September, unless it was paid within the last 6 months. *

For the time being, we will only be accepting payments by check.

All students are granted ONE free trial class; after this trial, students will not be able to participate in regular classes until payment is received by Dance Island.

MAKE-UP CLASSES: No tuition adjustments can be made for absences.

WITHDRAWAL: Dance Island should be notified in writing two weeks prior to withdrawal. You are responsible for two weeks of tuition payments after withdrawal notice, even if your child is not attending.

BEFORE AND AFTER CLASS: Dance Island, Laura Kunimura LLC and the Recreation Center staff cannot be held responsible to supervise children waiting before and after their class. Students who come before their scheduled class should observe and warm up quietly. Students who arrive more than 10 minutes late for their class can observe, but not participate in class.

I therefore release Laura Marlowe Kunimura, Laura Kunimura LLC, Dance Island, its teachers, and the Mililani Town Association (MTA) of any responsibility for accidents.
