The Rainbow Tree Connection, LLC - Home-care Agency

This is a new business but perhaps can be considered an extension of Rainbow Tree Therapies, LLC. This business will serve young adults ages 18-26 years old to promote leisure and social activities in the community, to promote overall health and wellness, and to promote basic independence skills.

Vision: The Rainbow Tree Connection recognizes the need for support of young adults with developmental disabilities who are seeking to maximize their independence. The Rainbow Tree Connection is a home care agency aiming to meet this need with high value within a community of connection and support. The vision is for the consumers and clients of The Rainbow Tree Connection to have the support needed to reach a maximum level of independence and to have a sense of belonging within their communities through engagement in leisure and wellness activities.

Mission: The Rainbow Tree Connection supports young adults in developing foundational independent living skills, health and wellness routines and community based social engagement skills to feel not only more confident but to also have a sense of purpose, connection and inclusiveness within their communities. We work with caregivers to develop a plan while acknowledging the importance of the client's voice in determining a supportive plan. We use a strengths based approach to maximize engagement while also determining interests/leisure activities that give meaning and purpose.

Our ideal client is a young adult who requires some support to engage in activities of daily living including social engagement. This client is not completely dependent but rather emerging in some skills. At this time, our ideal client has minimal to no challenges with intense aggressive behaviors and is not considered medically fragile. This client may be emerging in skills like establishing a wellness routine (getting adequate exercise and outdoor time), preparing a simple meal, doing light household chores, and finding leisure activities at home and in the community to participate in. *

The funding source for this service will be the responsibility of the primary caregivers. Payment will be required each month. At this time, this is not funded by medicaid waiver and you would need to contact your social worker to determine if funds could be used for this service. *
