I have enrolled in a program offered by Spotlight Dance Academy LLC. I, for myself, my heirs, or my assigns, release Spotlight Dance Academy LLC, the owner/director, Erin M. Fisk, her family, employees, helpers, and the owner of 202 Moravian Valley Road Suite C Waunakee, WI 53597, from any claims, demands, and causes of action from my or the above named person participating in Spotlight Dance Academy LLC. I also release them from any liability now or in the future for any injuries at Spotlight Dance Academy LLC or at any time on the premises or in any activity sponsored, represented, or organized by Spotlight Dance Academy LLC. By signing this agreement, I hereby affirm that I have read and fully agree with the above waiver and studio policies.
