• I confirm that the information given in the Health Questionnaire is correct at the time of completion and if any new condition develops during the season, I will inform the club’s Welfare Officer as appropriate. I give my permission to pass the above details to the club’s current insurance company if required.

• I give my permission for First Aid treatment to be administered if required. I understand that when participating in any sport there is a possibility of physical injury. Additionally, although the club is taking precautionary measures in accordance with guidance from Volleyball England (including limiting attendance at training sessions), there remains a possibility of transmission of coronavirus or other illnesses. I acknowledge that participation is at my own risk and agree to release and discharge the club from any and all claims.

• I agree to pay the Membership Fee and relevant Training/Match Fees.

• I confirm that I have read the club’s COVID-19 Training Plan and Risk Assessment and will follow the guidance stated in these documents.

• I confirm that I have read the club’s Code of Conduct and understand the behaviour expected as a member of Reading Aces Volleyball Club. I will encourage all participants within the club to have a “sportsman like” attitude, and to have fun in the pursuit of excellence in volleyball.

• I agree to the use of team photographs that include me /my child or children on the Reading Aces website and in other club publicity. I understand that the club will adhere to the Volleyball England Photography Guidelines to ensure the photos are safe and respectful and used solely for the purposes for which they are intended. If you have any additional information which you wish to share with the club regarding photographs, please contact your coach or the club’s Welfare Officer.

Additional declarations for Parents/Guardians only:

• I confirm that I have read the Safeguarding & Protecting Young People Policy and will support the aims of the club.

• I understand that the club, including the coaches, cannot accept any responsibility in relation to the transportation of junior club members to and from matches, training etc. The club will not be registering any private vehicles for the transportation of juniors. It is entirely the responsibility of parents/carers to ensure that they arrange suitable transport for the child/children in their care.
