Thank you for participating in my research project, this research project is revolved about injury in sport. this google form will be a series of questions linked to your age, gender, ethnicity, how often you take part in sport, what injury you sustained, how long you were out of the physical activity, what treatment and what you thought of the treatment. You will need to answer the questions honestly so we can get the most reliable results possible. The question answer boxes will range from tick boxes to short answers. 

Your data and answers will be stored and used in compliance with GDPR. All information will remain anonymous and no emails or other identifying information will be collected.
