BY COMPLETING THIS FORM, YOU ARE REGISTERING YOURSELF/YOUR DANCER AS A STUDENT OF MORGANTOWN DANCE, but you are not registering for any specific classes.  This form is to be used for TRYING A CLASS before deciding to register, OPEN CLASSES, DROP-IN CLASSES, or DANCE CARD CLASSES, and does register you for, hold your space for, or hold you financially responsible for any specific classes.

FOUNDATIONS & GROWN-UP & ME CLASSES CANNOT BE USED AS DROP-IN/DANCE CARD CLASSES.

There is a $15 registration fee to complete this form and attend the types of classes/events listed above.  This form remains VALID & IN FULL FORCE AND EFFECT from the date it is completed until you provide a written request to cancel it.

Completion of this form allows the dancer to attend class they want to try prior to registering, open classes, drop-in classes, or dance card classes that have available space.
REGISTRATION REQUIREMENTS & FEES
Every person who TRIES a class to see if they want to register, takes a DROP-IN/DANCE CARD, or attends an OPEN Class or MASTER class, MUST COMPLETE this Waiver & Liability form and pay the $15 registration fee  before they will be permitted to attend any classes.

NOTES ABOUT AGES & CLASS LEVELS
ALL OF THE AGES LISTED FOR CLASSES ARE GUIDELINES AND ARE NOT STRICTLY ADHERED TO IF THE INSTRUCTOR BELIEVES THE DANCER'S TALENT AND ABILITY PLACES THEM IN A DIFFERENT LEVEL THAN THAT IDENTIFIED BY THE DANCER'S AGE ONLY.  

All MDS , ages 8 & up (and any 7 year olds who want to see if they are ready for Non-Foundations classes), are required to register for placement for Ballet, Pointe, AcroFlex, HipHop, Irish, Jazz, Modern, and Tap, unless you are in Ballet II/III or up, Pointe II, AcroFlex III, Irish III, Irish Hardshoe II, Jazz III, Modern III, YOU WILL NOT ATTEND a live placement session.

FOUNDATIONS AND GROWN UP & ME CLASSES
Foundations & Grown Up & Me Classes may be tried prior to full registration, but may not be attended as a drop-in/dance card class.
NOTES ABOUT PLACEMENT CLASSES & BALLET LEVELS/BELT
You do not have to attend placement classes to attend as a drop-in/dance card student, but if you attend a class that is not believed to be the correct level for you by the instructor, you may be asked to attend a different class when you return again.
BALLET DRESS CODE
For all non-Foundations Ballet classes, the dress code is a Black Leotard, Pink Tights, and Pink Ballet Shoes.
TUITION & FEES
YOU MUST PAY THE $15 NON-REFUNDABLE REGISTRATION FEE.

Students cannot register for individual classes during the Fall & Spring classes, but can attend some classes (NOT FOUNDATIONS CLASSES) as a drop-in student by paying the night of the class or by pre-purchasing a dance card for anticipated drop-in classes.  Dance cards are good to be redeemed for 1 year from the date of purchase.  You may only drop-in, by paying or using a dance card, to a class that is NOT FULL.

0.25 hour class (15 minutes)           $4 per class
0.50 hour class (30 minutes)           $8 per class
0.75 hour class (45 minutes)           $11 per class
1.00 hour class (60 minutes)           $16 per class
1.25 hour class (75 minutes)           $19 per class
1.50 hour class (90 minutes)           $23 per class
1.75 hour class (105 minutes)         $26 per class
2.00 hour class (120 minutes)         $30 per class

DANCE CARD CLASS COSTS
0.25 hour class (15 minutes)           $3 per class
0.50 hour class (30 minutes)           $7 per class
0.75 hour class (45 minutes)           $10 per class
1.00 hour class (60 minutes)           $15 per class
1.25 hour class (75 minute)             $18 per class
1.50 hour class (90 minutes)           $22 per class
1.75 hour class (105 minutes)         $25 per class
2.00 hour class (120 minutes)         $29 per class

All male dancers ages 7 & Up MAY ATTEND ballet classes for FREE and all other dance genre classes for a half-price discount in all NON-Foundations classes.

PAYMENT OPTIONS & REQUIREMENTS
Based on the tuition charts above, you may pay class fees prior to attending classes or pre-purchase a dance card prior to attending classes.

Any class not paid for as a drop-in rate or by a pre-purchased dance card PRIOR to attending class(es) will result in an additional charge to the dancer/family.  

Any payment made after the 16th of each month will have a 10% late fee assessed.

You may choose to pay by cash, check, or credit/debit card at the Studio, or you may choose to pay online by PayPal invoice(s) using a credit/debit card or a PayPal account.

You may inquire about your balance by sending an email to mtowndance@gmail.com or asking at the front desk.  You will receive a response as soon as possible.
