This is a request to cancel your membership.  We are so sad to see you go! Are you sure you wish to cancel? We can freeze your membership for 30, 60 or 90 days! If you’d prefer to freeze go back to the main page and click on “Request to Freeze.”

- Your account/membership will be canceled 30 days from the date this form was submitted. If your autopay is scheduled during the 30 day cancellation period, your payment will be processed as scheduled and is nonrefundable.
