Name of the person who will receive the Gift Certificate  *
The studio is currently accepting students ages 12 and up.  If the student is under the age of 12, please let us know so that we can refer you to a studio that works with young voices.   *
Once this form is submitted, we will send you a Stripe Invoice for the amount.   *
Once the Stripe Invoice is paid, you will receive a Gift Certificate in that amount over email (it will have the name of the person you purchased it for on the gift certificate) - you will then need to forward the Gift Certificate to the person you purchased it for.  It will be in a PDF - so you can also print it if you wish.

Once the person receives their Gift Certificate, they will need to contact Monitz Vocal Studio to get on the studio schedule.  *
The Gift Certificate is good through Summer Session 2024.  Deadline to set up and schedule a voice lesson (or voice lessons) using their Gift Certificate is June 2024 - this gives us plenty of time to make sure they get their lesson(s) by the end of Summer 2024.    *
Availability at the Studio will vary throughout the year  - make sure to contact Monitz Vocal Studio right away to get on the schedule.  *
