I/We give our consent to send news, updates, events and special projects etc. from Omkara Aacadey of Fine and Performing Arts.

I/We here by give consent to my child/my being, filmed, interviewed, photographed or have audio or video recordings made of my child/me by the media (print, broadcast and on-line), and employees, agents of Omkara Academy of Fine and Performing Arts. Such may be used to report, publicize and / or promote programs in print and by electronic means for this and subsequent years.

I understand that the text or image(s) may appear in the electronic form on the internet or in other publications outside of Omkara Academy of Fine and Performing Arts’s control. I agree that I will not hold the Omkara Academy of Fine and Performing Arts responsible for any harm that may arise from such unauthorized reproduction.

I understand clearly that by agreeing to this release I will be forever prevented from suing or otherwise claiming against Omkara Academy of Fine and Performing Arts, its Director(s), Instructor(s), employee(s), agent(s), member(s) for any loss or damage connected with property loss or personal injury that me/ my ward may sustain while participating in the program, whether or not such loss or injury is caused solely or partly by the NEGLIGENCE of Omkara Academy of Fine and Performing Arts, its Directors, Instructors, employees, agents, members.

I understand that I/my ward will not be allowed to participate in the program unless this RELEASE WAIVER AND ASSUMPTION OF RISK AGREEMENT is signed. By agreeing to this document i approve that I am of sufficient age and mental capacity to sign this RELEASE WAIVER AND ASSUMPTION OF RISK AGREEMENT and understand all the points stated in the agreement and in my complete senses am agreeing to this document.

I am freely and voluntarily executing this RELEASE WAIVER AND ASSUMPTION OF RISK AGREEMENT.
