First Communion Registration Form
TO REGISTER YOUR CHILD, PLEASE SEE THE FOLLOWING INSTRUCTIONS:

1-Please read the following 3 documents: Parent Information Letter, Important Dates and Inscription Form found on the First Communion webpage: https://www.holyfamilywhitby.ca/first-holy-communion/
2- Complete the following online form and press SUBMIT to complete the application.
3- You will receive an email confirmation PDF, please print it.
4- Please bring the PDF confirmation along with a copy of your child's Baptismal Certificate and the Registration Fee of $50 (cash or cheque, payable to Holy Family.) to the parish office during office hours to complete the registration. We suggest you call ahead as office hours have been reduced at this time 905-665-6470.

NOTE: THERE IS A $50 REGISTRATION FEE, PAYABLE ON REGISTRATION NIGHT.
