Military Recruiting Opt-Out 2022 - 2023
IMPORTANT NOTICE REGARDING MILITARY RECRUITERS

As set forth in the 2015 Every Student Succeeds Act, Section 8025, PHS is required to provide military recruiters with the name, address and phone number of all Junior and Senior students unless a parent has 'opted out'.  If you wish to opt out, you must provide us written documentation asking us not to provide your student's directory information to the recruiters.  Please reply through this form asap.

Completion of this form will be considered your written notification of your choice to "opt-out" of the disclosure of your child's personal information.
