Assumption of Risk and Waiver of Liability

I recognize that attendance and participation may expose me, or my child, to risk of injury or harm. I accept this risk and agree that Upstate Carolina Dance Center and its staff will not be held responsible should such injury or harm occurring on the premises of UCDC or any UCDC Event. I give my consent to render appropriate judgment should my child have an accident or need medical attention and I am unable to be reached.

I hereby authorize the Upstate Carolina Dance Center to use photographs of the student on its website and in any publicity materials.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Upstate Carolina Dance Center or participation in Upstate Carolina Dance Center programming. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Upstate Carolina Dance Center, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Upstate Carolina Dance Center, its employees, agents, and representatives.
