16+ Camper Self Check In Permission
All campers must be signed in by parent/ guardian on the first day of camp.

Parental check in on the first day of camp will only be waived for students 16 and older who are driving themselves to camp when a parent/guardian completes and submits the form below PRIOR to the first day of camp.***

16+ campers are NOT permitted to check in younger siblings on the FIRST day of camp -- Younger campers must be checked in by parent/guardian on the first day.

16+ campers may check in/out their younger siblings on subsequent days of camp as long as they have the required PICK UP CARDS for their younger siblings (grades K-5).

***Families are responsible for the insurance and liability of their own vehicle, and ExploreHope assumes no responsibility for insurance or liability for this transportation.  ExploreHope will not be responsible for the coordination of additional passengers in this vehicle.
