FAN Weekly Activity Challenge Submission Form
Please use this form to submit your minutes of daily activity for the last two weeks.  Please only submit the number of minutes.  Do not convert into hours.  Please refer to the following examples for the correct and incorrect way to record your submission.  Correct entry: 75.  Incorrect entry: 75 minutes.  Incorrect entry: 1 hour and 15 minutes.  This form will begin accepting submissions the last Sunday of the challenge at 12am and will close Monday at 11:59pm.  You will have 48 hours to submit your results. No late submissions will be accepted.

Eligible activities are walking, running, jogging, biking, hiking, yoga, weight training, dancing, kayaking, skiing, skating, gardening, baseball, basketball, tennis, any sport, etc.  Essentially, anything that gets you moving for your health!  Three winners will be announced each month - one every two weeks and an over all monthly winner.  Winners will be announced in the e-blast following submission of results.  Good luck!
