Student Achievement Nomination Form
The student achievement award is designed to publicly honor students who have demonstrated exceptional achievement in one of the following categories:
I. Academics
II. Exemplary service to others
III. Character
IV. Citizenship
Who is eligible?
Any Franklin City Schools’ student, including Franklin students attending the Warren County Career Center.
What do students receive?
Students receive a State Achievement pin, a personalized Franklin City School District Student Achievement certificate, board and public recognition, and media coverage.
Who can submit a nomination?
Nominations may be submitted by fellow students, teachers, Board Members, counselors, principals, bus drivers, cooks, aides, librarians, etc. Anyone associated with Franklin City Schools who are aware of student achievement may submit a nomination.
Who makes the award decision?
A District Student Achievement Team, which includes students, parents, community members, Board members, and school administrators meets monthly during the school year. At each meeting, the team will consider any nominations that have been submitted.
When are awards given?
Awards will be presented at School Board meetings, 6 p.m. on the (usually) fourth Monday of the month in the Board room at Hampton Bennett.
