In exchange for the opportunity to participate, I release Saint Ignatius High School, its trustees, officers, agents, employees and volunteers, from any claims or causes of action for any personal injury or damage to personal property in connection with my participation in this activity. I understand that this Release covers any and all claims, including but not limited to negligence and or failure to supervise. I also understand that this Release binds me, my family, estate, and/or heirs.

Further, if any of the above named participant(s) are minor(s) for whom I am the parent or legal guardian, I certify that those individuals have my permission to participate in the above-described activity. For minors (under 18), choosing "I agree" to this form means that you have parental/guardian consent to participate.

Photos will be taken at the service sites. This waiver also serves as your permission to use images of you and/or your family for event promotion and similar purposes.

By signing below I acknowledge that my/my child’s participation in Spring Into Action may involve me/my child coming in contact with others, including but not limited to Saint Ignatius High School staff, volunteers, and other participants and their parents/guardians.

I also acknowledge and understand that, while Saint Ignatius High School is taking protective precautions to try to prevent exposure to, or the spread of COVID-19 and other viral or bacterial disease or illness, Saint Ignatius High School cannot guarantee that me/my child or other participants will not be exposed to such a condition. Saint Ignatius High School is following the Mandatory procedures outlined by the State of Ohio, and is implementing the Recommended Best Practices where practical.

Having considered the information available and the possible risks involved, I authorize my child to participate in Spring Into Action, and I agree to waive any and all claims against Saint Ignatius High School, its volunteers and the others associated with Spring Into Action for any injuries, illness or damages that may result from exposure to, or contracting COVID-19 or any other viral or bacterial disease or illness while participating in the program.
