As the parents/guardians of the child to be registered I hereby consent to his/her participation in the Basketball Saskatchewan Program(s). I/we do hereby release Basketball Saskatchewan, Canada Basketball, referees, volunteers, agents and/or employees, other players or participants, sponsoring agencies, sponsors, advertisers and if applicable, owners and leasers of premises used to conduct the program from all liability in respect to any injury sustained by my son/daughter while participating in the Basketball Saskatchewan program(s) and related activities.

I authorize Basketball Saskatchewan and/or its partners to use my or my child's photo for reproduction in any manner Basketball Saskatchewan desires, for advertising, display, audio-visual, exhibition or editorial use.

I understand the refund policy as stated below.

BSI Refund Policy:

Tournaments Prior to the start of the tournament : Registration fees will be refunded less a $50 Administration Charge. From two weeks - 14 days - before the start of the Tournament: There will be a 50% refund. From a week - 7 days - before the start of the Tournament: There will be no refunds.
