This is to put forth your interest in entering one of the Wetaskiwin Minor Hockey Tournaments. Filling this out does NOT confirm your spot in a tournament. A tournament coordinator will contact you with a formal invitation if your team is chosen.  If something changes you can always come back and adjust your answers or create a new entry. Teams chosen based on compatibility and are NOT picked in the order that they apply.
