Parents/guardians, please fill out this form completely. Students are NOT allowed to complete their own form, even if they are 18. Please include your phone number so the school office staff or administration can contact you with any questions.
Thank you for filling out this form for all late arrivals, absences, or early departures on a school day, including partial day absences. You can use the same form for all of your students. Parents/guardians are not required to email or call the office in addition to completing this form, but are welcome to reach out at anytime.
This form cannot be used for pick up changes. If you need to have your student picked up by someone other than you, please contact your campus office directly.
By completing this form, you agree that you have read and understand attendance policy as outlined in the ACA handbook. It is your responsibility to connect with teachers to make sure all missed work is made up in a timely manner.
