Girl Fight Fitness requires the completion of this form at least 28 days before your next bill in order to cancel your membership.   We will not process any cancellation requests made via email, phone, or text without the completion of this form.  This is in order to properly reconcile your account and to cancel any upcoming payments per our 28-day-notice policy.
