This end of the year, comprehensive program survey is open to all participants of YMH programming and will take 5 to 20 minutes to complete.

It is suggested that guardians and players work thru the questions together to provide honest feedback for our Association to consider.

A new survey should be completed for each team you are evaluating.

The survey can be submitted anonymously, however, if you would like someone to discuss any of your comments with you, you will have to provide a name and contact number.
