To allow the PHS Committee to reach you for this event, administrative or operational purposes
   
(B) Consent to Provide Personal Data
By indicating your consent to provide personal details, you agree to be contacted by the PHS Committee strictly with regards to upcoming PHS health screening.

(C) Withdrawal of Consent
Should you wish to withdraw your consent for the committee to contact you, please notify us in writing to medsocphs@gmail.com. We will remove your information from our database within 5 working days.
