I hereby grant to Emmanuel Chinese Baptist Church (ECBC) the right and unrestricted permission to use my children’s name, likeness, image, voice, and/or appearance as such may be embodied in any photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of the ECBC. I agree that ECBC has complete ownership of such material and can use said material for any purpose consistent with ECBC’s mission. These uses include, but are not limited to, videos, publications, advertisements, news releases, Websites, and any promotional or educational materials in any medium.  ECBC will NOT sell the images for commercial purposes. I acknowledge that I will not receive any compensation for the use of such images, video, likeness, etc. I hereby release and discharge ECBC, and its agents, representatives and assignee from any and all claims and demands arising out of or in connection with the use of my children’s name, likeness, image, voice and/or appearance, including any and all claims for invasion of privacy, right of publicity, misappropriation or misuse of image, and/or defamation. My signature below indicates that I have read, fully understand, and freely signed this agreement, which gives up any legal rights and or remedies which may be available to me or my child for the ordinary negligence of ECBC or any affiliated person. I affirm that I am of legal age and am freely signing this agreement.
