Church Membership and Visitor Form

This form is intended primarily to help the members at Northside Baptist Church in Slidell connect with one another through a member registry, although visitors are welcome to use this form as a method of contact as well. Please submit one form per adult member, even if some of the information overlaps. This information will be used to create an individual profile in Churchteams for every member at Northside. This form should take about 5–6 minutes to complete.

As a means of keeping our membership registry updated, you can use this form to "check in" periodically throughout the year. At the end of every year, as long as you or someone from your household has checked in at least once in a calendar year, then we will keep you and your family registered as a member.
