            HPS is exploring the possibility of creating a PHASE II of our Restart Plan.  As such, please fill out the following information if you would be interested in having your child’s status (Remote-Only/Hybrid) changed to EVERYDAY IN SCHOOL LEARNING.  If you want your child to remain remote-only or stay with the weekly hybrid plan then you will not need to fill in this form.

We can’t guarantee that your request will be granted, however we need this data so that we can make the best possible decisions when creating a PHASE II of our Restart Plan.


This form is due October 1, 2020 at 11:59 P.M.


You must fill in this form for each child that you have at HPS individually.  


If you want your child to remain remote-only or stay with the weekly Hybrid plan then you will not need to fill in this form.  This form is only for parents that are requesting EVERYDAY IN SCHOOL LEARNING.
