Consent Agreement
I, the parent/guardian of the above-named minor child, in consideration for my child being permitted to attend "Feed My Starving Children" in Chanhassen, MN with the 3SaintsYouth program on February 26th, from 6:00 - 8:00 p.m.

I hereby give my permission for said child to attend said event and agree to indemnify and hold harmless, to the furthest extent permitted by law, the Archdiocese of St. Paul and Minneapolis, Ss. Peter & Paul Catholic Church, the City of Loretto, MN, St. Thomas the Apostle Catholic Church, the City of Corcoran, MN, and any and all of its/their employees, supervisors, and/or volunteers from any harm, claim for damages, cause of action, lawsuit or other legal proceeding resulting from the injury or death of my child, (and/or for myself if I attend said the event also).

Further, in the event of injury to the child, and I cannot be reached, I hereby grant authorized medical professionals the right to administer necessary emergency treatment to my child.

Media Consent
Should photos or video be taken, I give my permission for the use of the image and/or likeness of my child in any promotional or other marketing activities relating to the event/activity or our parish Youth Ministry without compensation to me or my child.

If you do not want your child’s image and/or likeness to be used to promote parish youth ministry events, contact the parish office to receive a version of this form that does not include the previous clause; however, some events/ activities may require this clause.

Feed My Starving Children Covid-19 Protocols
All volunteers will be required to wear masks both inside the building at all times and outside the building when waiting to check-in. Acceptable masks include multi-layer cloth, non-medical disposable, or N95 that fit snugly and completely cover your nose and mouth. Please note that neck gaiters and plastic face shields that are worn alone are not acceptable. A mask with a face shield is acceptable.

All volunteers will have their temperature checked and go through a verbal symptom screening upon arrival. Temperatures must be under 100.4°F in order to pack.
Please arrive 10-15 minutes early to check-in and shop FMSC MarketPlace™.  If there is a line at the door, please wait until those in line have entered the building to help respect social distancing requirements.
Volunteer group size is limited to five people. Please note that you will not be socially distanced from anyone you choose to sign up with. You will be socially distanced from any other groups in the packing room. If you sign up as an individual, you will likely be paired with another volunteer but will still be six feet from each other (at opposite ends of our packing stations).

If your group is larger than five, simply register for multiple groups of five. Your groups of five will still need to maintain social distance from your other groups of five, however, we will do our best to keep your packing stations as close to each other as possible.

If you or anyone in your group develops symptoms while packing, you and your entire group will be asked to leave immediately.
