If you would like to file a commendation or complaint against a URI Police Officer, please fill out this form. Personal information will not be disclosed to the public, unless required by law. You can submit this form electronically. If you do not wish you submit the form electronically, you can request a paper copy at the URI Police station location at 85 Briar Lane, Kingston RI 02881.

If an officer was helpful, you can submit this form and it will be reviewed by supervisors to recognize a job well done. You can also call 401-874-2109 to make an anonymous commendation or complaint if you prefer not to use your name. Anonymous complaints will be investigated to the fullest extent possible.

If the complaint involves the Director of Public Safety, the complaint should be made directly to the Office of the Vice President for Administration and Finance, Carlotti Admin Building, Room 108, 75 Lower College Rd, Kingston, RI 02881.

If you require assistance in filling out this form, students should contact the Dean of Students Office at 401-874-2101. Faculty and Staff can call the Office of Human Resource Administration at 401-874-5270.

Recognition of professionalism: Involves what might be perceived as a small issue where the officer provided excellent service

Nomination for an award: Involves a serious situation where an officer went above and beyond the call of duty and should be considered for an award at the Annual Awards Ceremony

Inquiry: Involves what might be perceived as a minor complaint or concern, and only I want my complaint/concern on record. I understand it will be for informational purposes only, and may not be formally investigated. However, the matter will be at minimum be discussed with the employee(s) involved.

Formal Complaint: Involves a serious allegation of misconduct for which discipline may be imposed if the allegation(s)are sustained, and I want my complaint officially investigated.
