Recital/Convocation Request
Use this form to request a collaborative pianist and/or placement on a recital/convocation program. This form should be submitted as early as possible, but no later than four weeks prior to a convocation performance or seven weeks prior to a junior/senior recital.
Piano scores should also be submitted to the faculty collaborative pianist, Dr. Kumiko Shimizu, by this date. If the piano score and your part do not have printed measure numbers, please write them into both parts at the beginning of each line.
