A separate form should be completed for each student riding the bus.

Preschool transportation and special education transportation should be arranged through the special education department at the board office-please do not complete this form if you have transportation set up in an IEP.

Please complete this form only if your student will be riding the bus to and/or from school.  If you do NOT have a student that will be riding the bus to or from school this year, you may exit this form now to continue with the Skyward school enrollment.
