The Athlete Development Program Application Form [herein referred to as 'Form'] collects information that is considered private and confidential. The information collected in this Form will be used to facilitate the Athlete Development Program [ADP] selection process. The Google Forms platform is used to collect the information in accordance with the Google Terms of Use and Privacy Policy. Access to the information collected will be overseen by the ADP Manager. The information will be accessed (1) by those involved in administering the selection process and (2) by the student's host school for verification purposes and (3) if requested by external entities in accordance with relevant regulations and legislation. By completing the form you confirm and acknowledge that:(1) You have read the collection and privacy information (above) which outlines why the data is being collected, how it will be used, and who it will be accessible to. (2) You are the parent/guardian of the student applying for ADP selection (or an adult legally authorised to do so).
