Letter of recommendation request form
You need to follow instructions carefully. Please be sure to do the following:
  • Answer the questions below
  • Send me all links for letter uploads and any emails that require letters IN ONE GO if at all possible.  I will batch submit letters to all places in one go.  In general, I may not accept another letter request made less than 2 months after this request, so please have all of your requests ready to go at once.
If I feel unable to write you a letter, I will give you a reason why. Although I accept most requests for letters, when I do not accept it is usually because of one of two reasons: 1) the request was made less than four weeks before the earliest due date; or 2) I do not feel I know the person well enough to write a strong letter. Even if a student receives a good grade in a course, it can be hard to write an effective letter for a student that one doesn't know. A letter that simply says "This student received a good grade in my class" is not actually a good letter. Please consider this before requesting a letter of me. If you think there are other faculty who know you better, please consider asking them instead.
