Please use this form to submit changes to an EXISTING CLUB currently listed on the MHS Clubs/Activities & Advisors spreadsheet. Ensure you make these changes to your Club Information Sheet in Google Docs as well. If you cannot edit your Club doc, check if you are logged in to Google with your school gmail. If you still cannot edit, request access through Google Docs.
